Androgen-dependent conditions
CPA is used in the treatment of prostate cancer, precocious puberty, androgen-dependent skin and hair conditions such as acne, seborrhea, hirsutism (excessive hair growth), and androgenic alopecia (pattern hair loss), hyperandrogenism (e.g., in PCOS), and to reduce sex drive in sex offenders or men with paraphilias or hypersexuality.CPA is also widely used as a component of hormone replacement therapy (HRT) for transgender women.In the U.S., where CPA is not available, other drugs with antiandrogen properties like the diuretic spironolactone and the progestin medroxyprogesterone acetate are used instead to treat androgen-dependent conditions besides prostate cancer.

In the treatment of acne in women, a formulation of low-dose CPA in combination with EE has been found to result in overall improvement in 75 to 90% of patients, with responses approaching 100% improvement.

Hormonal contraception
The combination of CPA and EE, a formulation sometimes referred to as co-cyprindiol, has been available as a combined oral contraceptive since 1978.[24] This formulation is taken once daily for 21 days, followed by a 7-day free interval.[citation needed] CPA has also been available in combination with estradiol valerate (brand name Femilar) as a contraceptive in Finland since 1993.

How cyproterone acetate works?

Hormones are substances produced naturally in the body. They act as chemical messengers and help control the activity of cells and organs. Hormonal therapies interfere with the way hormones are made or how they work in the body.

Most prostate cancers need the hormone testosterone to grow. Almost all testosterone in men is made by the testicles. A very small amount is made by the adrenal glands, which sit above the kidneys.

Cyproterone acetate reduces the amount of testosterone made by your body. This reduces testosterone levels and may shrink the prostate cancer or stop it growing.

Contraindications:

Must not be used in patients with meningioma or a history of meningioma.
Hypersensitivity to the active substance or to any of the excipients listed in section 6.1.
Liver disease (including Dubin-Johnson syndrome and Rotor syndrome)
Malignant or wasting diseases (except for prostatic carcinoma for which cyproterone acetate is specifically indicated)
Previous or existing liver tumors (only if these are not due to metastases from carcinoma of the prostate)
Existing thromboembolic processes or a history of thromboembolic disorders
Immaturity: cyproterone acetate should not be given to youths, usually those under 18 years of age, whose bone or testicular maturity is incomplete.
Should not be used by females. Cyproterone acetate, used alone, is absolutely contraindicated in women of child-bearing age.

How to Take Cyproterone acetate?

Cyproterone is available as an oral (by mouth) tablet and as a long-acting injectable. The recommended dose for cyproterone tablets is 200 mg to 300 mg (4 to 6 tablets) daily, divided into 2 to 3 doses and taken after meals.

For men who have had an orchiectomy (removal of testicles), a lower dose of 100 mg to 200 mg daily in divided doses is recommended. The recommended dose for the long-acting form of cyproterone (Androcur® Depot) is 300 mg in injected into a muscle once a week.
