History:

Formestane was the first selective aromatase inhibitor to be developed as a prescription drug, first appearing in Europe during the mid-1990s under the Lentaron Depot brand name. It was sold by Novartis, which marketed Lentaron Depot in two-dozen countries including Argentina, Austria, Belgium, Brazil, Canada, Chile, Czech Republic, Denmark, France, Germany, Greece, Hong Kong, Ireland, Israel, Italy, Malaysia, Netherlands, Portugal, South Africa, Spain, Switzerland, Turkey, and the United Kingdom. With the emergence of newer and more effective aromatase inhibitors, however, formestane soon lost market presence at a rapid rate. Most of the initial Lentaron Depot preparations have since been discontinued. The drug remains available today, but only in a small number of nations. This includes Austria, Brazil, Czech Republic, Hong Kong, and Turkey.

Applications:

1. Treatment of Breast Cancer:

Because of its potent estrogen-suppressing action, formestane has been used clinically to treat breast cancer patients in a number of countries including England, Germany, Switzerland, Spain, Australia, New Zealand, Italy, and Malaysia. It has been shown to be an effective option as a second line of defense after tamoxifen, an estrogen receptor antagonist, has failed to elicit a positive response with patients, and produces an overall response statistically similar to tamoxifen when administered as the first-line therapy. In terms of overall potency, formestane is not as strong as the selective third generation inhibitors like Arimidex (anastrozole) or Femara (letrozole).

2. Use for Bodybuilding:

Formestane can lower SHBG up to 34%, which will increase free testosterone levels. SHBG (sex hormone binding globulin) is a protein that binds to sex steroids, and renders them inactive, for as long as SHBG is bound to them. There is a difference between 'free' testosterone, and 'bound' testosterone. Free testosterone is what matters, so lowering SHBG will increase 'free' testosterone. Higher testosterone means more strength, muscle mass, less body fat, better sense of well being, and libido.

Side Effects:

The side effects which accompany Lentaron use are also those that are common to other AI drugs including fatigue, mood swings, depression, joint pain, hot flashes, headaches, swelling of the arms and legs, and high blood pressure. AIs also suppress HDL (good) cholesterol levels, and may decrease bone density increasing susceptibility to osteoporosis, fractures and breaks.

Form and Dose:

Lentaron is available as an intramuscular depot injection, and is often found in 2 ml ampules of 125 mg/ml solution. The recommended dosage for bodybuilder purposes is the same as that for advanced breast cancer therapy, namely 250 mg per week. Formestane can also be found, and taken in the same 250 mg per week dosage in its oral form, but the oral had poor bioavailability is no longer a popular version of this drug.
