Medical Use:

1. Benign prostatic hyperplasia:

Physicians use finasteride for the treatment of BPH, informally known as an enlarged prostate. Finasteride may improve the symptoms associated with BPH such as difficulty urinating, getting up during the night to urinate, hesitation at the start of urination, and decreased urinary flow. It provides less symptomatic relief than alpha-1 blockers such as tamsulosin and symptomatic relief is slower in onset (six months or more of treatment with finasteride may be required to determine the therapeutic results of treatment). Symptomatic benefits are mainly seen in those with prostate volume > 40 cm3. In long-term studies finasteride but not alpha-1 inhibitors reduce the risk of acute urinary retention (−57% at 4 years) and the need for surgery (−54% at 4 years). If the drug is discontinued, any therapeutic benefits reverse within about 6-8 months.

Finasteride works by blocking the conversion of the hormone testosterone to the more potent hormone dihydrotestosterone. It does this by blocking an enzyme called 5-alpha reductase. Dihydrotestosterone is partly responsible for making the prostate enlarge. Therefore, a reduced amount of dihydrotestosterone in the prostate causes it to shrink. This helps you to pass urine more easily.

Hair loss caused by male pattern baldness occurs when a hormone called DHT (dihydrotestosterone) prevents hair follicles from getting the vital nutrients they need to live. Finasteride blocks the production of DHT and thus protects the hair follicles that produce hair. Reducing DHT can stop and even reverse hair loss caused by male pattern baldness.

3. Off-label uses:

Finasteride is sometimes used in hormone replacement therapy for transgender women in combination with a form of estrogen due to its antiandrogen properties. However, little clinical research of finasteride use for this purpose has been conducted and evidence of efficacy is limited.

How to Take Finasteride:

Take finasteride exactly as your doctor tells you to. Take one 5 mg tablet each day.

You can take the tablet either before or after a meal, but try to take it at the same time of day each day. This will help you to remember to take your doses regularly.

If you forget to take a dose on time, take it when you remember. If you do not remember until the following day, skip the missed dose. Do not take two doses together to make up for a forgotten dose.
