Tibolone (brand name Livial, Tibofem), also known as 7α-methylnoretynodrel, works by mimicking the activity of the female sex hormones, oestrogen and progesterone. It also has some male hormone (androgen) effects. Tibolone helps to restore the balance of female hormones in women who have a lack of oestrogen; it helps to ease symptoms such as hot flushes and night sweats.Tibolone was introduced in 1988 and is used widely in Europe, Asia, Australasia, and, with the exception of the United States (where it is not available), the rest of the world. Tibolone has similar or greater efficacy compared to older hormone replacement drugs, but shares a similar side effect. It has also been investigated as a possible treatment for female sexual dysfunction.

Tibolone is also prescribed to help prevent osteoporosis (bone loss). It is only suitable for women who have been through the menopause, and providing there have been at least 12 months since the last natural period. It is prescribed when other, more preferred, treatments are not suitable.

Background:

Tibolone (tye' boe lone) is a synthetic estrogen-like steroid hormone which acts as an agonist of multiple type I steroid receptors and thus has weak estrogenic and progestational activities as well as some androgenic properties. The parent compound has no activity, but undergoes tissue-specific metabolism via a sulfatase to an active form which provides estrogenic effects without the need for concurrent progesterone administration. Tibolone also appears to have reduced estrogenic activity in breast tissue. It has been shown to be effective against the symptoms of menopause and in prevention of postmenopausal osteoporosis; it is approved in several European countries, but not the United States. Tibolone is available as 1.25 and 2.5 mg tablets under the brand name Livial. Side effects include vaginal bleeding, abdominal pain, weight gain, bloating, breast tenderness, genital pruritus and vaginitis.

Do Not Take Tibolone if:

If you have or have ever had breast cancer, or if you are suspected of having it
If you have cancer which is sensitive to oestrogens, such as cancer of the womb lining (endometrium), or if you are suspected of having it
If you have any unexplained vaginal bleeding
If you have excessive thickening of the womb lining (endometrial hyperplasia) that is not being treated.
If you have or have ever had a blood clot in a vein (thrombosis), such as in the legs (deep venous thrombosis) or the lungs (pulmonary embolism)
If you have a blood clotting disorder (such as protein C, protein S, or antithrombin deficiency)
If you have or recently have had a disease caused by blood clots in the arteries, such as a heart attack, stroke or angina
If you have or have ever had a liver disease and your liver function tests have not returned to normal
If you have a rare blood problem called “porphyria” which is passed down in families (inherited)
If you are allergic to tibolone or any of the other ingredients of this medicine (listed in section 6)
If you are pregnant or think you might be pregnant.
If you are breastfeeding.

When to Use Tibolone:

Tibolone is suitable for women who have not experienced a natural period for at least 1 year. If taken sooner, irregular bleeding may be experienced.
Usually a woman using cyclical HT and experiencing a monthly bleed can start tibolone after the oestrogen/progesterone phase of her current HT.
Postmenopausal women not currently taking HT, or who have had a hysterectomy, may start at any time.
A woman taking HT that contains the same amount of oestrogen and progestogen every day of the month can change to Tibolone at any time.

Side-effects of tibolone:

Side-effects are uncommon but may include headache, dizziness, nausea, abdominal pain, swollen feet and itching. Breast tenderness is also uncommon..

Slight bleeding or spotting may commonly occur initially but tends to subside after a few months. Amenorrhea is achieved by about 80% of women after the first month of treatment with tibolone and over 90% after the third month of therapy.

Dose:

Take one 2.5 mg tablet every day. You may take tibolone at whatever time of day you find easiest to remember, but try to take your doses at the same time of day, each day. You can take the tablets before or after meals. You may find it helps to swallow the tablet with a drink of water.
If you forget to take a dose, take it as soon as you remember unless you are more than 12 hours late. If you are more than 12 hours late, leave out the forgotten dose and take your next tablet at the usual time. Do not take two tablets together to make up for a missed dose.

How to Store Tibolone:

Keep all medicines out of the sight and reach of children.
Store in a cool, dry place, away from direct heat and light.
